Job Description

A Thrive Team Leader at Rehoboth Day Program, supports Rehoboth Citizens by assisting with daily living skills, facilitating activities, promoting independence, documenting progress, managing behaviors, and fostering a safe, inclusive environment. This role emphasizes a faith-based approach to care, fostering an environment of compassion, respect, and spiritual growth. In essence, as a Thrive Team Leader, you would be responsible for creating an environment for our Rehoboth Citizens to thrive.

Responsibilities:

  1. Team Leaders participate in appropriate activities relating to the overall care and development of the Rehoboth Citizens including but not limited to coordination, implementation, monitoring and evaluation of individual service plans.
  2. Implements individual service plans by providing training to Rehoboth Citizens in specific performance areas, including but not limited to activities of daily living, food planning and preparation, money management, spiritual growth, recreation and leisure time.
  3. Maintains and submits timely and accurate data/reports regarding the developmental status, progress and other related information concerning the Rehoboth Citizens. This includes but is not limited to daily goal data, summaries and other reports as required.
  4. Must participate in in-service training programs and staff meetings as required. Failure to attend in-service training and/or staff meetings will result in disciplinary action up to and including termination of employment.
  5. Organizes, assists, supervises and, when necessary, prepares meals for Rehoboth Citizens.
  6. Ensures a safe and clean environment for Citizens at all times.
  7. Assists with drop-off and dismissal time for Rehoboth Citizens.
  8. Records and submits Rehoboth Citizen incident reports per established Rehoboth policy.
  9. Appropriately communicates to staff and director, both verbally and in writing (i.e. incident reports), pertinent Citizen and/or program issues ensuring the continuity of care and efficient operation of the Rehoboth Citizens.
  10. Transport Rehoboth Citizens to and from planned outings.
  11. Performs related duties assigned by the Rehoboth Director.

Requirements:

A minimum of a high school diploma and one year work experience in a program for the intellectually/developmentally disabled.

Skills and Abilities:

  • Possess good time management skills
  • Ability and desire to work as part of an interdisciplinary team
  • Moderate level of written and oral communication skills
  • High level of interpersonal skills
  • Knowledge of behavioral techniques and developmental disabilities to assist in the care of individuals receiving services
  • Knowledge of computers and ability to utilize word processing programs
  • First Aid will be provided upon hire

Reporting Relationship:

Thrive Team Leaders report to and are supervised by the Rehoboth Director.
